Prognostic significance of calibrated integrated backscatter in patients submitted to transcatheter aortic valve implantation

Background There is a reported association between calibrated integrated backscatter (cIB) and myocardial fibrosis, mostly noted in cardiomyopathies aortic stenosis cases. But whether this measure ultrasound reflectivity could help predict outcomes patients submitted to transcatheter valve implantation has not been previously tested. Purpose To demonstrate cIB may differentiate undergoing (TAVI) at risk major cardiovascular events. Methods Consecutive transfemoral TAVI October 2020 December 2021 were routinely imaged by echocardiography the five days peri-procedure. Calibrated was obtained from parasternal long-axis view (PLAX) subtracting pericardial intensity septal posterior wall average cIB. Patients with poor PLAX views excluded study. All measurements performed end-diastole. The primary endpoint occurrence events one year follow-up. Results A total 184 underwent procedure during study period. Of these, 159 had reasonable acoustic window for measurement included Patient mean age 81.1±6.1 years, 58% female. At baseline, 81% preserved left ventricle function, gradient 44.8±15.1 mmHg, Euroscore II 5.55±3.75. No significant found TAVI-related acute complications (21.02±10.4 vs 19.8±7.1 dB, p = 0.544). also no correlation (19.71±7.25 20.39±10.47 0.478) or safety (19.59±7.07 18.84±10.32 0.709), defined accordingly Valve Academic Research Consortium-2 consensus, one-year was, however, positive all-cause mortality (19.24±7.06 22.92±9.36, 0.021) follow-up, deaths occurring due multiple causes, mainly cardiovascular, infectious, neoplastic. Measured significantly higher pre-TAVI estimated glomerular filtration rate less than 30 mL/min/1.73m2 (22.42±7.17 19.56±6.88 0.043). Conclusion Higher associated TAVI, but it can potentially have prognostic value differentiating increased mortality.
